Wrist Blood Pressure Monitors

These are automatic, compact, and convenient. They wrap around the wrist and use the same oscillometric method as upper arm cuffs. The problem is position. Your wrist contains the radial artery, which is smaller and more sensitive to arm position than the brachial artery. If your wrist is even slightly below heart level during the reading, the number can be meaningfully off.

Wrist monitors are not useless. Some people with larger or unusually shaped upper arms find them more practical. But they require strict technique, and fewer wrist models appear on validated device lists.

Smartwatches (PPG-Based Monitoring)

How to Take Your Blood Pressure the Right Way

These steps apply to any automatic upper arm monitor. Getting these right matters more than which brand you buy.

Step 1: Rest quietly for 5 minutes first. No phone, no conversation, no TV. This is the most skipped and most important step. A reading taken immediately after walking to the chair can be 10 to 15 points higher than one taken after five minutes of stillness.

Step 2: Avoid these for at least 30 minutes before measuring. Caffeine, exercise, smoking, a heavy meal. A full bladder alone adds up to 15 mmHg to your systolic reading.

Step 3: Sit in the correct position. Back against a supported chair. Feet flat on the floor, never crossed. Crossed legs raise systolic pressure by 2 to 8 mmHg. Rest your arm on a flat table at heart height. If your arm is below heart level, the reading goes up. If it is above, it goes down.

A single reading, or even a week of once-daily readings, is not as useful to your doctor as a structured series.

The clinical standard is the 7-day home monitoring protocol. Seven days. Two sessions per day: morning and evening. Two to three readings per session with one minute between each. Discard all readings from day one because the first day tends to run higher due to unfamiliarity with the device. Average everything from days two through seven.

6 Mistakes That Make Your Readings Unreliable

Wrong cuff size. A cuff that is too small overestimates blood pressure by 2 to 10 mmHg. Measure your mid-upper arm circumference and match it to the cuff size chart included with your device. A smartwatch that takes a reading every 30 minutes throughout the day gives you something a twice-daily cuff session cannot: the full arc of your blood pressure across an entire day. You can see whether readings consistently spike in the afternoon. You can see that your numbers are higher on days when sleep was poor. You can notice that a stressful week correlates with elevated readings. None of that is visible with two manual measurements per day.

For clinical accuracy, they are not there yet. PPG-based readings from a smartwatch have measurement variability that is too high for medication decisions. Can I take my blood pressure immediately after waking up?
